One of our most requested services is providing a support plan for individuals moving into a new residential care service. We have years of experience working with families, support workers, healthcare professionals and local authorities to ensure continuity of care for a supported person. Our team of experienced PBS practitioners will conduct a comprehensive assessment, identifying the person's strengths, communication preferences, and any areas of concern related to potential behavioural challenges during the transition. We will then translate these findings into easy-to-understand reports that outline the individual's needs and recommend helpful support strategies.

We offer a range of PBS training options to equip staff at both the current and new service with the necessary skills and knowledge. This can include general PBS workshops, training specific to the individual's needs and direct coaching to ensure all staff are prepared to provide consistent and effective support.

Seamless Transition Plans

We will develop clear and actionable transition plans that outline positive behaviour support strategies for the new service provider. These plans will detail communication methods, routines and de-escalation techniques that are already in place for the individual. This ensures continuity of care and minimises the risk of challenging behaviours upon arrival at the new home.
